Description:
Celecoxib is a selective COX-2 inhibitor, classified as a nonsteroidal anti-inflammatory drug (NSAID). It provides anti-inflammatory, analgesic, and antipyretic effects with a lower risk of gastrointestinal complications compared to non-selective NSAIDs.
Indications:
Celecoxib is indicated for the treatment of:
- Osteoarthritis (OA)
- Rheumatoid arthritis (RA)
- Ankylosing spondylitis
- Acute pain (e.g., postoperative, musculoskeletal pain)
- Primary dysmenorrhea (menstrual pain)
Dosage & Administration:
Adults:
- Osteoarthritis: 200mg once daily or 100mg twice daily
- Rheumatoid arthritis: 100-200mg twice daily
- Ankylosing spondylitis: 200mg once daily or 100mg twice daily
- Acute pain & Dysmenorrhea: Initial dose 400mg, followed by 200mg if needed on the first day, then 200mg twice daily as needed
Elderly Patients (≥65 years):
- No dosage adjustment needed, but lower doses should be considered in patients with low body weight (<50kg)
Patients with Hepatic Impairment:
- Moderate impairment (Child-Pugh Class B): Reduce dose by 50%
- Severe impairment: Use is not recommended
Patients with Renal Impairment:
- Use with caution in patients with severe renal disease (CrCl <30 mL/min)
Administration:
- Can be taken with or without food
- Swallow capsules whole with water
- For patients who have difficulty swallowing, the capsule contents may be mixed with applesauce
Contraindications:
- Hypersensitivity to Celecoxib, sulfonamides, or other NSAIDs
- History of severe allergic reactions (e.g., anaphylaxis, angioedema) to NSAIDs
- Active peptic ulcer disease or gastrointestinal bleeding
- Severe hepatic or renal impairment
- Coronary artery bypass graft (CABG) surgery-increased risk of cardiovascular events
Precautions:
- Cardiovascular Risk: May increase the risk of heart attack, stroke, and hypertension, especially with long-term use
- Gastrointestinal Risk: May cause gastric ulcers, bleeding, or perforation, especially in elderly patients
- Renal Effects: May cause fluid retention, hypertension, and kidney dysfunction-monitor renal function
- Use in Pregnancy: Avoid in the third trimester-may cause fetal harm
Adverse Reactions:
- Common: Nausea, dyspepsia, diarrhea, headache, dizziness, peripheral edema
- Serious: Hypertension, heart failure, gastrointestinal bleeding, renal dysfunction, Stevens-Johnson syndrome
Drug Interactions:
- Anticoagulants (Warfarin, Heparin): Increased bleeding risk-monitor INR closely
- Diuretics & ACE Inhibitors: May reduce effectiveness-monitor blood pressure and kidney function
- Other NSAIDs & Corticosteroids: Increased risk of gastrointestinal bleeding
- Fluconazole: May increase Celecoxib levels-consider dose reduction
Storage Conditions:
- Store below 25°C, in a dry place
- Protect from light and moisture
