It is used to treat essential hypertension. This fixed-dose combination is indicated for the treatment of patients whose blood pressure is not effectively controlled by irbesartan or hydrochlorothiazide alone.
Matters needing attention:
Hypotension - hypovolemic patients: This compound is used in hypertensive patients with no other risk factors for hypotension and is rarely associated with symptomatic hypotension. Symptomatic hypotension may occur in patients with insufficient blood volume and sodium due to the use of strong diuretics, severe salt restriction in the diet, and diarrhea and vomiting. These conditions should be corrected before treatment with this compound.
Renal artery stenosis - Renal vascular hypertension: Patients with bilateral renal artery stenosis in a single functioning kidney who are treated with angiotensin-converting enzyme inhibitors or angiotensin II receptor antagonists may be at risk for severe hypotension and renal insufficiency. Although this has not been found in studies of this product, this effect should be taken into account when used.
Renal impairment and kidney transplantation: Regular monitoring of serum potassium, creatinine, and uric acid is recommended when using this product in patients with renal impairment. There is no experience with the use of this product in patients who have recently undergone kidney transplantation. Patients with severe renal insufficiency (creatinine clearance < 30ml/min) (see contraindication) should not use this product. Azotemia associated with thiazide diuretics may occur in patients with renal impairment. For those with renal insufficiency with creatinine clearance ≥30ml/min, no dose adjustment is required. However, it should be used with caution in patients with mild to moderate renal impairment (creatinine clearance ≥30ml/min but < 60ml/min).
Liver function impairment: In patients with liver function impairment, the use of thiazide diuretics should be cautious because minor changes in fluid and electrolyte balance may trigger hepatic coma. There is no experience of using this compound in patients with liver function impairment.
Aortic and mitral stenosis, hypertrophic obstructive cardiomyopathy: As with other vasodilators, this product should be used with caution in patients with aortic and mitral stenosis and hypertrophic obstructive cardiomyopathy.
