Irbesartan and Hydrochlorothiazide Tablets (150mg/12.5mg)
Composition:
Each tablet contains:
- Irbesartan 150mg (Angiotensin II Receptor Blocker - ARB)
- Hydrochlorothiazide (HCTZ) 12.5mg (Thiazide Diuretic)
Description:
This fixed-dose combination consists of Irbesartan, an angiotensin II receptor blocker (ARB) that relaxes blood vessels, and Hydrochlorothiazide (HCTZ), a diuretic that helps eliminate excess salt and water from the body. The combination effectively lowers high blood pressure (hypertension) and reduces cardiovascular risks.
Indications:
Irbesartan and Hydrochlorothiazide Tablets are indicated for:
- Hypertension in patients not adequately controlled on Irbesartan or HCTZ monotherapy
- Patients requiring combination therapy for better blood pressure control
Dosage & Administration:
Adults:
- Starting dose: 150mg/12.5mg once daily
- Maintenance dose: 150mg/12.5mg or 300mg/12.5mg once daily
- Maximum dose: 300mg/25mg per day
Renal Impairment:
- Not recommended for patients with severe kidney disease (eGFR <30 mL/min/1.73m²)
Hepatic Impairment:
- Use with caution in moderate liver disease
- Not recommended in severe hepatic impairment
Elderly Patients:
- No dosage adjustment required, but monitor kidney function closely
Administration:
- Take once daily, preferably in the morning
- Can be taken with or without food
- Do not crush or chew
Contraindications:
- Hypersensitivity to Irbesartan, HCTZ, or excipients
- Pregnancy (Category D) - fetal toxicity risk
- Severe kidney or liver disease
- Anuria (inability to produce urine)
- Severe electrolyte imbalance (e.g., hypokalemia, hypercalcemia)
Precautions:
- Monitor blood pressure, kidney function, and electrolyte levels regularly
- Risk of dehydration and hypotension due to diuretic effect
- Hyperkalemia risk with potassium supplements or potassium-sparing diuretics
- Increased photosensitivity (use sunscreen when exposed to sunlight)
- Diabetic patients: May cause blood sugar fluctuations
- Gout patients: HCTZ may increase uric acid levels
Adverse Reactions:
- Common: Dizziness, headache, fatigue, muscle cramps, nausea
- Electrolyte Imbalance: Hypokalemia, hyponatremia, hypercalcemia
- Cardiovascular: Hypotension, palpitations
- Serious (rare): Angioedema, severe hypotension, kidney dysfunction
Drug Interactions:
- NSAIDs: May reduce antihypertensive effect and worsen kidney function
- Potassium-sparing diuretics (e.g., spironolactone): Risk of hyperkalemia
- Lithium: Increased risk of lithium toxicity
- Other antihypertensives: May enhance blood pressure-lowering effects
- Antidiabetic drugs: HCTZ may reduce effectiveness of insulin or oral hypoglycemics
Storage Conditions:
