Indications:
Prevention and treatment of endemic goiter, pre-operative preparation for hyperthyroidism and hyperthyroidism crisis.
Adverse reactions:
1. Allergic reactions, not very common. Angioedema may occur immediately after taking the drug, or several hours later, as edema of the upper extremities, lower extremities, face, lips, tongue, or larynx. Erythema or wind, fever, and discomfort may also occur.
2. Joint pain, eosinophilia, and lymph node enlargement are uncommon.
3. If taken for a long time, iodine poisoning symptoms such as burning sensation in the mouth and throat, salivation, metallic taste, gum pain, stomach discomfort and severe headache can occur. Hyperkalemia may also occur, characterized by confusion, arrhythmia, numbness and tingling in the hands and feet, and heavy weakness in the lower limbs.
4. Gastrointestinal adverse reactions such as diarrhea, nausea, vomiting and stomach pain are uncommon.
5. Periarteritis, leukemioid eosinophilia, rare.
Pharmacology and toxicology:
This product is iodine supplement. Potassium iodide can have two effects on thyroid function depending on the dose.
1. In the prevention and treatment of endemic (simple) goiter, a small dose of iodine preparation is given as a raw material to supply iodine to synthesize thyroxine, which corrects the excessive secretion of thyroid stimulating hormone in the original pituitary gland and shrinks the enlarged thyroid gland.
2. Large doses of iodine act as antithyroid agents to temporarily control hyperthyroidism. This may prevent the release of free thyroid hormones into the blood by inhibiting thyroglobulin hydrolase, which is quick and strong, but not long-lasting. Thyroid hormone synthesis is temporarily inhibited, and the inhibition can disappear after continuous administration, resulting in more severe hyperthyroidism symptoms. Therefore, it is only used in thyroid crisis to rapidly improve symptoms, and must be combined with thiourea drugs.
Large doses of iodine can also counteract the thyrotropin action of the pituitary gland, shrink and harden the thyroid tissue and reduce the blood vessels, so as to facilitate surgery. This effect reaches its peak in 2 weeks of medication, so it is advisable for patients with hyperthyroidism to take thiourea drugs for a period of time before surgery, so that symptoms and basal metabolic rate are basically controlled, and medication is started 2 weeks before surgery. The drug can also improve sudden eye symptoms, slow down the heart rate, reduce metabolic rate.
