Indications:

1. Herpes simplex virus infection: for the treatment of primary and recurrent mucosal skin infections in immunocompromised persons and for the prevention of recurrent cases; It is also used in the treatment of herpes simplex encephalitis.
2. Shingles: For the treatment of severe shingles in immunodeficient patients or diffuse shingles in normal immunofunctional patients.
3. Treatment of chickenpox in immunodeficient patients.

 

Usage and dosage:

For intravenous infusion only, each infusion time is more than 1 hour.

Normal adult dosage

1. For the initial treatment of severe genital herpes, 5mg/kg(according to acyclovir, the same below), 3 times a day, once every 8 hours, for a total of 5 days.

2. For immunodeficient patients with herpes simplex or severe herpes zoster, 5 to 10mg/kg per body weight, 3 times a day, once every 8 hours, for 7 to 10 days.

3. Herpes simplex encephalitis, 10mg/kg per body weight, 3 times a day, once every 8 hours for 10 days.

4. Acute retinal necrosis, 5 to 10mg/kg once, 3 times a day, once every 8 hours, a total of 7 to 10 days. After that, 0.8g was taken orally 5 times a day for 6 to 14 weeks.

The maximum daily dose for adults is 30mg/kg for body weight or 1.5g/m2 for body surface area.

Normal dosage for children

1. For the first treatment of severe genital herpes, infants and children under 12 years old were given 250mg/m2 once according to the body surface area (according to acyclovir, the same below), 3 times a day, once every 8 hours, for 5 days.

2. Herpes simplex of skin and mucous membrane in immunodeficient infants and children under 12 years old, 250mg/m2 once according to body surface area, 3 times a day, once every 8 hours, for 7 days, and above 12 years old according to adult dose.

3. Herpes simplex encephalitis, 10mg/kg per body weight, 3 times a day, once every 8 hours for 10 days.

4. For immunodeficient patients with chickenpox, 10mg/kg per body weight or 500mg/m2 per body surface area, 3 times a day, once every 8 hours, for 10 days.

The maximum dose for children is 500mg/m2 per body surface area every 8 hours.

Patients with acute or chronic renal insufficiency should not use this product for intravenous infusion, because the rapid rate of infusion can cause renal failure.

Preparation of liquid medicine: Take 0.5g of this product and add it to 10ml water for injection, make the concentration 50g/L, shake well to form a solution, and then dilute it with sodium chloride injection or 5% glucose injection to at least 100ml, so that the final concentration of the drug does not exceed 7g/L, otherwise it is easy to cause phlebitis.


Precautions:

1. This product contains amoxicillin, which is a penicillin-type drug. Intradermal sensitivity test of penicillin sodium should be conducted before using this product.

2. Cross-irritability exists between this product and other penicillins and cephalosporins.

3. When extending the course of treatment, liver and kidney function and blood image should be checked irregularly. Patients with gonorrhea should be tested for syphilis three months after initial diagnosis and treatment.

4. False positive results for urine glucose tests using Clinitest, Benedict solution, or Fehling solution after taking this product. It is recommended that this test be performed by a glucose oxidase reaction method (such as Clinistix or Tes-Tape) after use of this product.

5. When pregnant women use it, the plasma levels of bound estriol, estradiol - glucoside, bound estrone and estradiol will appear transient increase.

6. This product must be used in time after being prepared into a solution, and should not be stored for a long time.

7. This product should not be used by patients treated with Allopurinol or Disulfiram.

 

Medication for special population:

1. Warning:

Acyclovir can cause death when treated with kidney damage. Renal function should be checked before or during medication.

Thrombus formation, thrombocytopenic purpura, hemolysis, uremic syndrome (TT; /HUS) and can result in death.

2. Patients with acute or chronic renal insufficiency should not use this product for intravenous infusion, because the rapid rate of infusion can cause renal failure.

3. Special care should be taken when using acyclovir in patients receiving potentially nephrotoxic drugs, as this may increase the risk of renal dysfunction and increase reversible central nervous system symptoms.

4. People allergic to Ganciclovir may also be allergic to this product.

5. The pros and cons of the drug should be considered in the following cases: The dose of this product should be reduced in patients with dehydration. In patients with severe liver dysfunction, intolerability of this product, mental abnormality or past psychoreaction to cytotoxic drugs, the use of this product is easy to produce mental symptoms, and should be used with caution.

6. There is no research data on the initiation of treatment after 72 hours of acute onset of shingles, so patients diagnosed with shingles should be treated as early as possible.

7. Chickenpox: chickenpox is a self-limiting mild to moderate disease in healthy children and more severe in adolescents and adults. Treatment should be carried out within 24 hours of the acute onset of chickenpox, and there is no effective treatment data to start treatment at a later stage.

8. This product does not eradicate the virus for genital herpes infection. There was no obvious effect on latent infection and recurrence of herpes simplex virus. There is no information on whether this product prevents transmission of the disease to others. Because genital herpes is a sexually transmitted disease, patients should avoid contact with patients and refrain from sexual intercourse to avoid infecting their spouses. Genital herpes can also be transmitted in the absence of symptoms, passing through the asymptomatic virus. If a recurrence of genital herpes is found, the patient should be treated as soon as the first signs or symptoms are noticed. Since most people with genital herpes are prone to cervical cancer, patients should be tested at least once a year for early detection.

9. Long-term use of this product may cause drug resistance in patients with severe immune dysfunction. If the skin damage of herpes simplex patients does not improve after application of this product, the sensitivity of the virus to this product should be tested.

10. Intravenous infusion is slow, otherwise the drug crystallization and precipitation in the renal tubules may occur, causing kidney function damage up to 10% of the cases, and do not let it leak outside the blood vessels, so as not to cause pain and phlebitis.

11. 2 hours after intravenous infusion, urine drug concentration is the highest, at which time the patient should be given sufficient water to prevent drug deposition in the renal tubules.

12. One hemodialysis can reduce the concentration of blood drugs by 60%, so a dose should be repeated for 6 hours each hemodialysis.

13. The dose for obese patients should be calculated according to standard body weight. When dose adjustment is required, it can be adjusted according to creatinine clearance.

14. Interference with diagnosis: can cause renal tubule obstruction, increase blood creatinine and urea nitrogen. If the dosage is appropriate, adequate moisture generally does not cause.

15. This agent is alkaline, and it is easy to change the pH value when mixed with other drugs, and should be avoided as far as possible.

16. Patients should consult a doctor if they feel a serious or disturbing adverse reaction, if they are pregnant or trying to become pregnant, if they are preparing to breastfeed, or if they have other problems, and use drugs rationally under the guidance of a doctor.
