Pharmacology and toxicology:
There is growing evidence that dysfunction of glutaminergic neurotransmitters, especially when the function of NMDA receptors is impaired, presents clinical symptoms and disease progression of neurodegenerative dementia.
Memantine is a non-competitive NMDA receptor antagonist with moderate affinity and voltage dependence. It blocks neuronal damage caused by pathologically elevated glutamate concentrations.
Ocular changes have been observed in repeated dose toxicity tests in rodents and dogs, but no consistent conclusions have been reached, and these results have not been observed in monkeys. The specific eye exams performed in Memantine's clinical trial did not reveal any eye lesions.
No genotoxicity was observed at standard doses of Memantine. Lifetime studies in rats and mice did not find any evidence of carcinogenesis. In rats and rabbits, memantine is teratogenic and has no adverse effects on fertility, even at maternal toxic doses. In rats, delays in embryonic development were observed when memantine doses were equivalent to or slightly higher than human doses.
