Diphenhydramine Hydrochloride Injection 50mg/5mL (10 mg/mL)
Composition:
Each 5mL ampoule contains:
- Diphenhydramine Hydrochloride - 50 mg (10 mg/mL)
- Excipients: Sodium citrate, Citric acid monohydrate, Benzyl alcohol (as preservative), Water for injection
Description:
Diphenhydramine is a first-generation H1 antihistamine with anticholinergic, antiemetic, and sedative properties. It is used to treat allergic reactions, anaphylaxis (adjunct), motion sickness, and extrapyramidal symptoms.
Indications:
Diphenhydramine Injection is indicated for:
- Severe allergic reactions (including anaphylaxis adjunct therapy)
- Acute urticaria & angioedema
- Motion sickness & nausea
- Drug-induced extrapyramidal symptoms (EPS)
- Insomnia (short-term use when oral therapy is not possible)
Dosage & Administration:
Adults
- Allergic reactions: 10-50 mg IM/IV every 4-6 hours (Max 400 mg/day)
- Anaphylaxis (adjunct to epinephrine): 25-50 mg IV, followed by oral therapy if needed
- Motion sickness: 10-50 mg IM/IV 30 minutes before travel
- Extrapyramidal symptoms: 10-50 mg IM/IV every 6-8 hours
Pediatrics (≥2 years old)
- Allergic reactions: 1 mg/kg/dose IV/IM every 6-8 hours (Max 50 mg per dose)
- Motion sickness: 1 mg/kg IM 30 minutes before travel
- Extrapyramidal symptoms: 1 mg/kg IM/IV every 6-8 hours
Administration:
- Slow IV push over 2 minutes to minimize hypotension
- Deep IM injection into a large muscle (e.g., deltoid, gluteal)
Contraindications:
- Hypersensitivity to diphenhydramine or similar antihistamines
- Neonates & premature infants (risk of respiratory depression)
- Severe asthma attacks
Precautions:
- CNS depression risk: Avoid activities requiring alertness
- Elderly patients: Increased risk of dizziness, confusion, falls
- Use with caution in patients with:
- Glaucoma
- Prostatic hypertrophy
- Severe liver or kidney impairment
Adverse Reactions:
- Common: Drowsiness, dizziness, dry mouth, blurred vision
- Serious: Hypotension, tachycardia, paradoxical CNS stimulation (especially in children)
Drug Interactions:
- CNS depressants (alcohol, benzodiazepines, opioids): Increased sedation
- Monoamine oxidase inhibitors (MAOIs): Prolonged and intensified effects
- Anticholinergic drugs: Increased risk of urinary retention, dry mouth
Overdose & Management:
- Symptoms: Hallucinations, seizures, respiratory depression, anticholinergic toxicity
- Treatment: Supportive care, activated charcoal if early, benzodiazepines for seizures
