Indications:

This product is used for patients who do not respond well to oral iron and require intravenous iron treatment [1], such as: 

- Patients who cannot tolerate oral iron 

- Patients with poor oral iron absorption

 

Usage and dosage:

Usage 

This product can only be mixed with 0.9%w/v normal saline. This product should not be mixed with other therapeutic products. 

Visually inspect berkelium for precipitation and breakage before use. Only those that have not precipitated should be used. 

This product should be used immediately after the container is opened: 

If stored in daylight at a temperature of 4 ° C to 25 ° C, the product diluted with 0.9% normal saline should be used within 12 hours. 

This drug should be administered intravenously by drip or slow injection, or directly into the venous end of the dialyzer. It is not suitable for intramuscular injection or a single full dose of the total amount of iron required by the patient. 

Before the first treatment in a new patient, a small dose should be administered as recommended, 1-2.5ml(20-50mg) iron for adults, body weight > 1ml(20mg iron) for 14 kg children, weight < Half the daily dose (1.5mg/kg) for children of 14 kg. Cardiopulmonary resuscitation equipment should be available. If no adverse reactions occur after 15 minutes of administration, continue to administer the remaining liquid. 

Infusion: 

The preferred method of administration of this product is by drip (to reduce the risk of hypotension and intravenous injection). 1ml of this product can only be diluted into a maximum of 20ml 0.9%w/v saline, and the diluent should be used immediately after preparation (e.g. : 5ml of this product can be diluted into a maximum of 100ml 0.9%w/v saline, and 25ml of this product can be diluted into a maximum of 500ml 0.9%w/v saline). The infusion rate of the liquid medicine should be: 100mg iron for at least 15 minutes; Drop 200mg for at least 1.5 hours; Drop 400mg for at least 2.5 hours; Leave 500mg for at least 3.5 hours. 

If clinically necessary, the diluent volume of 0.9% normal saline of this product can be less than a specific amount to form a higher concentration of this product. However, the rate of infusion must be determined according to the dose of iron given per minute (e.g. : 10ml of this product =200mg of iron should be dropped in at least 30 minutes; 25ml of this product =500mg of iron should be finished in at least 3.5 hours). In order to ensure the stability of the liquid medicine, it is not allowed to mix the liquid medicine into a thinner solution. 

Intravenous injection: 

The recommended rate is 1ml of the product per minute (5ml of the product for at least 5 minutes), and the maximum dose per injection is 10ml of the product (200mg of iron). After intravenous injection, the patient's arm should be extended. 

To inject into the dialyzer: 

This product can be injected directly into the venous end of the dialyser, the situation is the same as the previous intravenous injection.

Calculate the total iron deficiency according to the following formula. To determine the dosage for each patient: 

Total iron deficiency [mg]= Body weight [kg]x(Hb target value -Hb actual value)[g/l]x0.24*+ stored iron [mg] 

Weight ≤35kg: Hb target value =130g/l stored iron =15mg/kg body weight 

Weight > 35kg: Hb target value =150g/l stored iron =500mg 

* Factor 0.24= 0.0034x0.07x1000 

(Hemoglobin content is about 0.34%/ blood volume is about 7% of body weight/Factor 1000 refers to the conversion from g to mg)

Matters needing attention:

Rare allergic reaction. 

Occasionally, ≥1% of the following adverse reactions have been reported: metallic odor, headache, nausea, vomiting, diarrhea, low blood pressure, elevated liver enzymes, cramps/leg cramps, chest pain, lethargy, dyspnea, pneumonia, cough, itching, etc. 

Parasympathetic excitation, gastrointestinal dysfunction, muscle pain, fever, rubella, facial flushing, limb swelling, dyspnea, varicose veins, and venous spasm at the site of infusion have been rare.
