Indications:

This product is mainly used to assist general anesthesia, ease tracheal intubation and skeletal muscle relaxation during surgery.

 

Usage and dosage:

Vecuronium bromide is for intravenous use only.

With or without vecuronium during the intubation period, the following doses may be used as reference for the first dose and maintenance dose to meet the muscle relaxation requirements of various procedures under balanced anesthesia.

Intubation dose: Vecuronium bromide 0.08-0.1mg/kg.

The first dose required after endotracheal intubation with succinylcholine: 0.03-0.05mg/kg. If succinylcholine is used for intubation, use this product after the clinical effect on the patient has subsided.

Maintenance dose: 0.02-0.03mg/kg. It is best to add a maintenance dose when the twitch height returns to 25% of the control value.

As with other neuromuscular blocking drugs, the dosage should vary from patient to patient. In addition, the method of anaesthesia, the duration of the operation, the effects of other drugs used before or during the anaesthesia operation and the patient's condition all need to be considered. A peripheral nerve stimulator should be used to monitor the degree of neuromuscular block and recovery.

Matters needing attention:

- In obese patients (defined as those whose weight is 30% or more above the ideal body weight), the dosage should be reduced and the ideal body weight calculation dosage should be used.

- Inhaled anesthetics do enhance the neuromuscular blocking effect of vecuronium. During anesthesia, this enhancement effect is clinically relevant only when the volatile anesthetics in the tissue reach the required concentration for dynamic interaction. Therefore, in surgeries where inhalation anesthetics are used for more than 1 hour, a lower maintenance dose of vecuronium should be given by lengthening the interval between administration of vecuronium or slowing down the rate of infusion.

- Where it is necessary to use higher doses on patients, for example in emergency surgery to prevent inhalation of stomach contents and when succincholine is prohibited, the initial dose of vicuronium bromide has been used under intraoperative conditions of simultaneous halothane and neuroepam anesthesia from 0.15mg/kg bw to 0.3mg/kg bw, There are no adverse cardiovascular effects (as long as proper ventilation is maintained). The use of higher doses of vecuronium shortened the onset of the strongest neuromuscular block after administration and extended the duration of drug action.

- The dose for caesarean section and neonatal surgery should not exceed 0.1mg/kg.

Because vecuronium bromide can cause respiratory muscle paralysis, ventilation support must be performed until the patient regains sufficient spontaneous breathing.
