Vancomycin Hydrochloride for Injection 1g
Composition:
Each vial contains:
- Vancomycin Hydrochloride equivalent to 1g of Vancomycin
Description:
Vancomycin is a glycopeptide antibiotic used primarily for Gram-positive bacterial infections, including methicillin-resistant Staphylococcus aureus (MRSA) and Clostridium difficile infections.
Indications:
Vancomycin Hydrochloride for Injection is indicated for the treatment of serious infections caused by susceptible Gram-positive bacteria, including:
- MRSA infections
- Pneumonia
- Septicemia (bacteremia)
- Endocarditis
- Bone and joint infections (e.g., osteomyelitis)
- Skin and soft tissue infections
- Clostridium difficile-associated diarrhea (oral use only)
- Meningitis (as part of combination therapy)
Dosage & Administration:
Intravenous (IV) Use Only
Administer slowly over at least 60 minutes to reduce the risk of Red Man Syndrome.
Adults & Children (>12 years)
- Usual dose: 15-20 mg/kg every 8-12 hours
- Severe infections: Loading dose 25-30 mg/kg, then 15-20 mg/kg every 8-12 hours
- Clostridium difficile colitis (oral use only): 125-500 mg every 6 hours
Neonates & Infants (<12 years)
- 0-7 days old: 10-15 mg/kg every 12 hours
- >7 days to 1 month: 10-15 mg/kg every 8 hours
- Children 1 month-12 years: 10-15 mg/kg every 6-12 hours
Reconstitution & Administration:
- Reconstitute 1g vial with 20mL sterile water
- Further dilute in 100-250mL normal saline or 5% dextrose
- Administer over at least 60 minutes via IV infusion
Contraindications:
- Hypersensitivity to vancomycin or glycopeptide antibiotics
Precautions:
- Monitor renal function (risk of nephrotoxicity, especially with aminoglycosides)
- Monitor hearing function (ototoxicity risk, particularly in elderly patients)
- Red Man Syndrome: Rapid infusion may cause flushing, rash, hypotension
Adverse Reactions:
- Common: Nausea, vomiting, fever, rash, flushing (Red Man Syndrome)
- Serious: Nephrotoxicity, ototoxicity, neutropenia, anaphylaxis
Drug Interactions:
- Aminoglycosides (e.g., gentamicin, amikacin): Increased nephrotoxicity
- Loop diuretics (e.g., furosemide): Increased ototoxicity
- Neuromuscular blockers: Enhanced neuromuscular blockade
Overdose & Management:
- Symptoms: Renal toxicity, severe hypotension, hearing loss
- Treatment: Supportive care, hemodialysis (if necessary)
Storage Conditions:
- Store below 25°C, protect from light
- Use reconstituted solution within 6 hours (if stored at room temperature) or 24 hours (if refrigerated)
