  • Pemetrexed Disodium for Injection (200 mg) is an antifolate chemotherapy agent used primarily to treat certain types of cancer, particularly in the lungs and pleura. It works by inhibiting folate-dependent enzymes critical for DNA and RNA synthesis, slowing down the growth and spread of cancer cells.

Indications:

Pemetrexed is commonly used for:

  • Non-Small Cell Lung Cancer (NSCLC): As a first-line or maintenance therapy, usually in combination with other chemotherapy agents like cisplatin.
  • Malignant Pleural Mesothelioma: In combination with cisplatin, for unresectable or metastatic cases.

Administration:

  • Infusion: Administered intravenously over 10 minutes.
  • Folic Acid and Vitamin B12 Supplementation: Patients typically take oral folic acid (400-1000 mcg daily) and receive intramuscular vitamin B12 (1000 mcg) injections every 9 weeks to reduce toxic effects.

Precautions:

  • Renal Function Monitoring: Pemetrexed is primarily excreted by the kidneys, so renal function should be assessed before each dose.
  • Pregnancy and Breastfeeding: Contraindicated due to potential harm to the fetus or infant.
  • Avoid NSAIDs: Nonsteroidal anti-inflammatory drugs (NSAIDs) may interfere with pemetrexed excretion, especially in patients with kidney impairment.
