Tranexamic Acid Injection 1g/10mL (100 mg/mL)
Composition:
Each 10mL ampoule contains:
- Tranexamic Acid - 1000 mg (100 mg/mL)
- Excipients: Water for injection
Description:
Tranexamic Acid is an antifibrinolytic agent that inhibits plasminogen activation, preventing the breakdown of blood clots (fibrinolysis). It is used to control excessive bleeding in various medical conditions and surgical procedures.
Indications:
Tranexamic Acid Injection is indicated for:
- Surgical bleeding prevention & control, including cardiac, orthopedic, liver, and gynecological surgery
- Trauma-related hemorrhage
- Heavy menstrual bleeding (menorrhagia)
- Hereditary angioedema
- Postpartum hemorrhage
- Bleeding disorders associated with fibrinolysis (e.g., leukemia, liver disease, hemophilia adjunct therapy)
Dosage & Administration:
Adults
- General bleeding control: 10-15 mg/kg IV every 6-8 hours
- Surgical prophylaxis: 1 g IV before surgery, followed by 1 g every 8-12 hours for up to 3 days
- Trauma hemorrhage: 1 g IV over 10 minutes, followed by 1 g over 8 hours
Pediatrics
- Surgical bleeding control: 10 mg/kg IV every 6-8 hours
- Menorrhagia (off-label): 10-25 mg/kg IV every 8-12 hours
Administration:
- Slow IV injection over 10 minutes to prevent hypotension
- Dilute in 100 mL of normal saline or dextrose for infusion
- Do not mix with blood products, penicillin, or solutions containing heparin
Contraindications:
- Active thromboembolic disease (DVT, PE, stroke, etc.)
- Severe renal impairment (risk of accumulation & toxicity)
- History of seizures associated with tranexamic acid use
- Hypersensitivity to tranexamic acid
Precautions:
- Use with caution in patients with cardiovascular disease or coagulopathies
- May increase the risk of seizures, particularly at high doses
- Monitor renal function in patients with kidney disease
- Avoid intra-arterial administration to prevent vascular complications
Adverse Reactions:
- Common: Nausea, vomiting, diarrhea, headache
- Serious: Hypotension (if given too rapidly), thrombosis, seizures, visual disturbances
Drug Interactions:
- Oral contraceptives & hormone therapy: Increased risk of thrombosis
- Anticoagulants (e.g., Warfarin, Heparin): May counteract effects
- Fibrinolytic agents (e.g., Alteplase): Reduced efficacy
Overdose & Management:
- Symptoms: Nausea, vomiting, dizziness, hypotension, seizures
- Treatment:
- Supportive care (monitor blood pressure & cardiac function)
- Seizure management with benzodiazepines
