Description:
Cimetidine is a histamine H2-receptor antagonist (H2-blocker) that inhibits gastric acid secretion. It is used in the treatment and prevention of peptic ulcers, gastroesophageal reflux disease (GERD), and other acid-related conditions.
Indications:
Cimetidine Injection is indicated for:
- Peptic ulcer disease (gastric and duodenal ulcers)
- Gastroesophageal reflux disease (GERD)
- Zollinger-Ellison Syndrome and other hypersecretory conditions
- Prevention of stress ulcers in critically ill patients
- Aspiration pneumonitis prophylaxis in surgical patients
Dosage & Administration:
Recommended Dosage (Adults):
- Peptic Ulcer Disease & GERD: 200 mg IV every 6 hours, or continuous infusion of 37.5 mg/hour
- Zollinger-Ellison Syndrome: 300 mg IV every 6 hours, adjust based on acid output
- Aspiration Pneumonitis Prophylaxis: 300 mg IV 1 hour before anesthesia
Administration:
- Intravenous (IV) administration only
- IV bolus: Dilute in 20 mL of 0.9% sodium chloride (NaCl) and administer slowly over 5 minutes
- IV infusion: Dilute in 100 mL of NaCl or 5% dextrose, infuse over 15-30 minutes
Dose Adjustments:
- Renal Impairment: Reduce dose in severe kidney disease (CrCl < 30 mL/min)
- Elderly Patients: Use lower doses as clearance may be reduced
Contraindications:
- Hypersensitivity to Cimetidine or other H2-blockers
- Severe renal or hepatic dysfunction (use with caution)
Precautions:
- Long-term use may lead to:
- Vitamin B12 deficiency
- Increased risk of pneumonia
- May cause gynecomastia with prolonged use
- Can interact with multiple drugs (inhibits CYP450 enzymes)
Adverse Reactions:
- Common: Headache, dizziness, diarrhea, rash
- Serious: Bradycardia, confusion (especially in elderly patients), liver enzyme elevation
Drug Interactions:
- Warfarin, Theophylline, Phenytoin: Increased drug levels-monitor closely
- Ketoconazole: Reduced absorption-take separately
- Procainamide: Increased risk of toxicity
Overdose & Management:
- Symptoms: Hypotension, confusion, bradycardia
- Treatment: Supportive care, IV fluids, symptomatic treatment
Storage Conditions:
- Store below 25°C, protect from light
- Use immediately after opening
