Description:
Paracetamol (also known as Acetaminophen) is a widely used analgesic (pain reliever) and antipyretic (fever reducer). It is effective for treating mild to moderate pain and fever, with minimal gastrointestinal side effects compared to NSAIDs.
Indications:
Paracetamol is indicated for the relief of pain and fever, including:
- Headache and migraine
- Toothache and dental pain
- Muscle pain and backache
- Arthritis and joint pain
- Menstrual pain (dysmenorrhea)
- Cold, flu, and fever symptoms
Dosage & Administration:
Adults and Children Over 12 Years:
- 500-1000mg (1-2 tablets) every 4 to 6 hours as needed
- Maximum daily dose: 4000mg (8 tablets) in 24 hours
Children (6-12 years):
- 250-500mg every 4 to 6 hours as needed
- Maximum daily dose: 2000mg (4 tablets) in 24 hours
Children Under 6 Years:
- Use only under medical supervision (consider liquid formulations)
Administration:
- Take orally with water
- Can be taken with or without food
- Do not exceed the recommended dose
Contraindications:
- Hypersensitivity to Paracetamol or any excipient in the formulation
- Severe liver disease or acute liver failure
Precautions:
- Liver Toxicity: Avoid exceeding the maximum dose, especially in patients with liver disease, chronic alcoholism, or long-term Paracetamol use
- Renal Impairment: Use with caution in patients with severe kidney disease
- Alcohol Use: Avoid excessive alcohol consumption, as it increases the risk of liver damage
- Use in Pregnancy and Breastfeeding: Generally safe, but should be used at the lowest effective dose for the shortest duration
Adverse Reactions:
- Common: Nausea, vomiting, rash
- Rare but serious: Liver toxicity, Stevens-Johnson syndrome, hematologic disorders (thrombocytopenia, neutropenia)
Drug Interactions:
- Alcohol & Hepatotoxic Drugs: Increased risk of liver damage
- Warfarin & Anticoagulants: Prolonged use may increase bleeding risk-monitor INR closely
- Rifampicin, Carbamazepine, Phenytoin: May reduce Paracetamol effectiveness by increasing metabolism
Overdose & Management:
- Symptoms of Overdose: Nausea, vomiting, abdominal pain, liver failure
- Treatment:
- Seek immediate medical attention
- Activated charcoal if within 1 hour of overdose
- N-acetylcysteine (NAC) is the antidote-administer as soon as possible
