Acyclovir for Injection 250mg
Composition:
Each vial contains:
- Acyclovir Sodium (equivalent to 250 mg of Acyclovir)
Description:
Acyclovir is a synthetic nucleoside analog antiviral drug that inhibits herpes simplex virus (HSV) DNA polymerase, preventing viral replication. It is effective against HSV-1, HSV-2, and Varicella-Zoster Virus (VZV) and is used for severe or systemic viral infections.
Indications:
Acyclovir for Injection is indicated for the treatment of:
- Severe herpes simplex virus (HSV) infections
- Herpes simplex encephalitis
- Neonatal herpes infections
- Varicella-Zoster Virus (VZV) infections, including herpes zoster (shingles) and severe chickenpox
- HSV or VZV infections in immunocompromised patients
Dosage & Administration:
Adults & Adolescents (≥12 years):
- Severe HSV infections: 5 mg/kg IV every 8 hours for 5-10 days
- HSV encephalitis: 10 mg/kg IV every 8 hours for 10-21 days
- Varicella-Zoster infections (shingles): 10 mg/kg IV every 8 hours for 7-10 days
Pediatric Patients:
- Neonatal herpes (≤28 days old): 20 mg/kg IV every 8 hours for 14-21 days
- Varicella in immunocompromised children: 10 mg/kg IV every 8 hours for 7-10 days
Renal Impairment:
- CrCl 25-50 mL/min: Administer every 12 hours
- CrCl 10-25 mL/min: Administer every 24 hours
- CrCl <10 mL/min: Administer half dose every 24 hours
- Hemodialysis: Give dose after dialysis
Administration Instructions:
- Reconstitution:
- Add 10 mL of sterile water for injection to the 250 mg vial
- Shake gently until fully dissolved
- Further dilute in at least 50-100 mL of 0.9% NaCl or 5% dextrose before infusion
- IV Infusion:
- Administer slowly over 1 hour to prevent kidney toxicity
- Do not administer as IV bolus or rapid injection
Contraindications:
- Hypersensitivity to Acyclovir, Valacyclovir, or excipients
- Severe renal impairment (use with caution, dose adjustment required)
Precautions:
- Ensure adequate hydration to reduce the risk of kidney damage
- Monitor renal function in elderly or dehydrated patients
- Neurological side effects (agitation, confusion, seizures) may occur in high doses
Adverse Reactions:
- Common: Nausea, vomiting, headache, dizziness
- Serious: Acute kidney injury (crystalluria), neurotoxicity (hallucinations, seizures), phlebitis at the injection site
Drug Interactions:
- Probenecid: Reduces renal clearance of Acyclovir-monitor for toxicity
- Mycophenolate Mofetil: Increases Acyclovir levels-use with caution
- Ciclosporin & Tacrolimus: Increases nephrotoxicity risk-monitor renal function
Overdose & Management:
- Symptoms: Renal failure, confusion, seizures
- Treatment: Hydration, symptomatic care, hemodialysis if necessary
Storage Conditions:
- Store below 25°C, protect from light
- Use reconstituted solution within 12 hours
