Parecoxib Sodium for Injection 40mg

Description:
Parecoxib is a selective COX-2 inhibitor (NSAID) used for the short-term treatment of postoperative pain. It is a prodrug of Valdecoxib, providing rapid pain relief while minimizing gastrointestinal side effects associated with traditional NSAIDs.
Indications:
Parecoxib Sodium is indicated for:
- Short-term management of acute postoperative pain
- Pain relief in patients unable to take oral NSAIDs
Dosage & Administration:
Adults:
- Initial dose: 40mg IV or IM
- Maintenance dose: 20-40mg every 6-12 hours as needed
- Maximum dose: 80mg per day
Elderly Patients (≥65 years):
- Initial dose: 20mg IV or IM, then 10-20mg every 6-12 hours
- Maximum dose: 40mg per day
Patients with Hepatic Impairment:
- Moderate impairment: Reduce dose to 20mg initially, then 10mg every 12 hours
- Severe impairment: Use is not recommended
Patients with Renal Impairment:
- Severe renal impairment (CrCl <30 mL/min): Use with caution
- End-stage renal disease: Use is not recommended
Administration:
- IV Injection: Reconstitute with 1mL of sterile water for injection and administer slowly over 15 seconds
- IM Injection: Reconstitute with 1-2mL of sterile water for injection and inject into a large muscle
- Do not mix with other drugs or solutions containing opioids
Contraindications:
- Hypersensitivity to Parecoxib, Valdecoxib, or other NSAIDs
- Severe hepatic impairment
- Active gastrointestinal bleeding or peptic ulcer disease
- History of coronary artery bypass graft (CABG) surgery
- Severe renal impairment (CrCl <30 mL/min)
Precautions:
- Cardiovascular Risk: May increase the risk of heart attack and stroke with prolonged use
- Gastrointestinal Risk: May cause gastric ulcers and bleeding-use with caution in patients with a history of GI disorders
- Renal Effects: Can cause fluid retention, hypertension, and renal dysfunction-monitor renal function in at-risk patients
- Avoid in Pregnancy: Not recommended, especially in the third trimester
Adverse Reactions:
- Common: Nausea, vomiting, headache, dizziness, injection site pain
- Serious: Hypertension, renal dysfunction, gastrointestinal bleeding, allergic reactions (including anaphylaxis)
Drug Interactions:
- Anticoagulants (Warfarin, Heparin): Increased bleeding risk-monitor INR closely
- Diuretics & ACE Inhibitors: May reduce effectiveness-monitor blood pressure and kidney function
- Other NSAIDs or Corticosteroids: Increased risk of gastrointestinal bleeding
- Methotrexate & Lithium: May increase toxicity-monitor drug levels
Storage Conditions:
- Store below 25°C, in a dry place
- Protect from light and moisture
- Use reconstituted solution within 6 hours if stored at room temperature
