Indications:
It is used to treat moderate to severe infections caused by these sensitive bacteria, including lower respiratory tract infections (pneumonia and bronchitis), in adults and children aged 2 months to 16 years. Empirical treatment of patients with simple lower urinary tract infections and complex urinary tract infections (including pyelonephritis), uncomplicated skin and skin soft tissue infections, complex intra-abdominal infections (including peritonitis and biliary tract infections), gynecological infections, sepsis, and neutropenia with fever. It can also be used in children with bacterial meningomyelitis.
Bacterial culture and susceptibility tests should be performed when bacterial infection is suspected, but because cefepime is a broad spectrum fungicide for gram-positive and gram-negative bacteria, cefepime monotherapy can be started before susceptibility test results are known. For suspected anaerobic bacteria mixed infection, it is recommended to use other anti-anaerobic drugs, such as metronidazole for initial treatment. Once the results of bacterial culture and drug susceptibility test are known, the treatment regimen should be adjusted in time.


Taboo:
This product should not be used in patients who have an immediate allergic reaction to cefepime or L-arginine, cephalosporins, penicillin or other beta-lactam antimicrobials.

Pharmacology and toxicology
Pharmacology: Cefepime is a broad-spectrum antibacterial drug, its mechanism of action is to inhibit bacterial cell wall synthesis. In vitro tests showed that cefepime was effective against both gram-positive and gram-negative bacteria. Cefepime has low affinity for β-lactamase encoded by bacterial chromosomes, is highly resistant to hydrolysis of most β-lactamases, and can quickly penetrate into gram-negative bacteria cells. The target molecule of cefepime in bacterial cells is penicillin-binding protein (PBP).
Cefepime has bactericidal effect on various bacteria. The MBC/MIC ratio of cefepime was less than 2 for more than 80% of gram-positive and negative test bacteria. Cefepime has synergistic effect with aminoglycoside antibiotics in vitro.
Cefepime against enterobacter in aerobic gram-negative bacteria, These include Enterobacter cloacae, Enterobacter aerogenes, Klebsiella pneumoniae, Serratia, Escherichia, Pseudomonas aeruginosa, Proteus Mirabilis, Proteus vulgaris, Enterobacter aggregatus, Citrobacter, Acinetobacter calacetoides, Haemophilus influenzae (including β-lactamase-producing strains) and Haemophilus Parainfluenza, Moraxella kataxella (including β-lactamase-producing strains), and Salmonella Bacillus, Hanifella honeybee, Morganella mogeni, Shigella, gonococci, etc., have strong antibacterial effects, and have good antibacterial activities against other Klebsiella bacteria except Klebsiella pneumoniae, and have certain antibacterial activities against other pseudomonas except Pseudomonas aeruginosa, alcaligenes, nitritrate negative bacilli. But stenotrophomonas (i.e., Pseudomonas maltophilia) is resistant to cefepime.
Cefepime has a good antibacterial effect on most gram-positive coccus, including streptococcus pneumoniae (including penicillin-resistant bacteria), hemolytic streptococcus, Streptococcus pyogenes, Streptococcus agalactis, Streptococcus virescens and other streptococcus, methicillin-sensitive Staphylococcus aureus, methicillin-sensitive staphylococcus epidermidis and other staphylococcus. But against methicillin-resistant staphylococcus less effective.
Most enterococcus, such as Enterococcus faecalis, and methicillin-resistant staphylococcus are resistant to cefepime.
Bacteroids in anaerobic bacteria (including melanogenes and other oral infections), Clostridium perfringens, Clostridium, Kinetobacter, peptostreptococcus, and propionibacterium are sensitive to cefepime, but fragile bacteroids and Clostridium difficile are resistant to cefepime.
