Indications:
High blood pressure:
This product is used to treat essential hypertension and renal vascular hypertension. It can be taken alone or in combination with other antihypertensive drugs.
Congestive heart failure:
This product can be combined with digitalis or diuretics as an adjunct treatment for congestive heart failure.
Acute myocardial Infarction:
It is used to treat patients with hemodynamic stability within 24 hours after acute myocardial infarction, and can prevent the development of left ventricular insufficiency or heart failure and improve survival. Patients should receive conventionally recommended treatments such as antisuppositories, aspirin, and beta-blockers when appropriate.
Drug overdose:
Symptoms of drug overdose can include severe hypotension, electrolyte disturbance, and kidney failure. After an overdose, patients should be closely monitored. Treatment is selected according to the nature and severity of symptoms. Measures to prevent absorption and accelerate excretion should be used. If severe hypotension occurs, the patient may be in a state of shock and should be quickly given intravenous saline. If possible, angiotensin II therapy may be considered. Angiotensin converting enzyme inhibitors can be cleared by hemodialysis. The use of high-throughput polyacrylonitrile dialysis membranes should be avoided. Serum electrolytes and creatinine should be monitored frequently.
Pharmacology and toxicology:
Lisinopril is a peptide dipeptidase inhibitor. It inhibits angiotensin-converting enzyme (ACE), which catalyzes the conversion of angiotensin I to a vasoconstricting peptide known as angiotensin II. Angiotensin II stimulates the secretion of aldosterone by the adrenal cortex. Inhibition of ACE can reduce the concentration of angiotensin II and thus decrease the pressor effect and aldosterone secretion. A decrease in the latter results in an increase in serum potassium.
Lisinopril mainly reduces blood pressure by inhibiting renin-angiotensin-aldosterone system, and also has antihypertensive effect on low renin hypertension. ACE is identical to kinin II, which degrades bradykinin, but whether increased blood levels of bradykinin (a vasodilating peptide) are associated with the antihypertensive effects of lisinopril remains unclear.
