Sugammadex Sodium Injection 200mg/2mL

Composition:

Each 2mL vial contains:

• Sugammadex Sodium - 200 mg
• Excipients: Sodium hydroxide and/or hydrochloric acid (for pH adjustment), Water for injection

Description:

Sugammadex is a modified γ-cyclodextrin that selectively binds to rocuronium and vecuronium, reversing their neuromuscular blocking effects. It is used to rapidly reverse neuromuscular blockade induced by these non-depolarizing muscle relaxants.

Indications:

Sugammadex is indicated for the reversal of neuromuscular blockade induced by:

• Rocuronium bromide
• Vecuronium bromide

It is used in adult and pediatric patients (≥2 years old) undergoing surgery where neuromuscular blockade reversal is required.

Dosage & Administration:

Adults:

The recommended dose depends on the depth of neuromuscular blockade:

Depth of Blockade	Sugammadex Dose	Expected Recovery Time
Moderate blockade (TOF* ≥2)	2 mg/kg IV	~1.5 minutes
Deep blockade (1-2 post-tetanic responses, TOF = 0)	4 mg/kg IV	~3 minutes
Immediate reversal (after rocuronium 1.2 mg/kg)	16 mg/kg IV	~3 minutes

(*TOF = Train-of-Four)

Pediatric Patients (≥2 years):

• Moderate blockade: 2 mg/kg IV
• Deep blockade: 4 mg/kg IV

Administration:

• Administer as a single IV bolus over 10 seconds
• Flush with saline after administration
• Do not mix with other drugs in the same IV line

Contraindications:

• Hypersensitivity to Sugammadex or excipients
• Severe renal impairment (CrCl <30 mL/min)

Precautions:

• Use with caution in patients with renal impairment (dose adjustment required)
• May reduce the effectiveness of hormonal contraceptives - advise alternative contraception for 7 days
• Monitor for recurrence of neuromuscular blockade in high doses
• Hypersensitivity reactions (including anaphylaxis) may occur - be prepared for emergency treatment

Adverse Reactions:

• Common: Bradycardia, hypotension, nausea, vomiting, cough
• Serious: Severe allergic reactions, anaphylaxis, recurrence of blockade

Drug Interactions:

• Rocuronium/Vecuronium: Sugammadex reverses their effects by direct binding
• Toremifene: May reduce Sugammadex effectiveness
• Contraceptives: Sugammadex may decrease hormonal contraceptive levels

Overdose & Management:

• Symptoms: Bradycardia, hypotension
• Treatment: Supportive care, IV atropine for severe bradycardia
