Propofol Injectable Emulsion 100mg/10mL
Composition:
Each 10mL ampoule contains:
- Propofol - 100mg (10mg/mL)
- Excipients: Soya bean oil, Glycerol, Egg lecithin, Sodium hydroxide (for pH adjustment), Water for injection
Description:
Propofol is a short-acting intravenous anesthetic used for induction and maintenance of general anesthesia and sedation. It provides rapid onset and smooth recovery, making it ideal for surgical and intensive care use.
Indications:
- Induction and maintenance of general anesthesia
- Sedation of mechanically ventilated patients in intensive care units (ICU)
- Procedural sedation for diagnostic and therapeutic procedures
Dosage & Administration:
Induction of General Anesthesia:
- Adults: 1.5-2.5 mg/kg IV bolus over 20-30 seconds
- Elderly & debilitated patients: Lower initial dose (1-1.5 mg/kg)
Maintenance of General Anesthesia:
- Continuous IV infusion: 4-12 mg/kg/hour
Sedation in ICU (Ventilated Patients):
- Initial dose: 0.3-4 mg/kg/hour IV infusion
- Titration: Adjust based on response
Procedural Sedation:
- Initial bolus: 0.5-1 mg/kg IV
- Maintenance infusion: 1.5-4.5 mg/kg/hour
Administration:
- IV use only
- Avoid rapid bolus administration in elderly patients
- Shake well before use
Contraindications:
- Hypersensitivity to propofol, soybean oil, or egg lecithin
- Patients with severe cardiac or respiratory compromise
- Pediatric sedation in intensive care (not recommended for prolonged use in children under 16 years)
Precautions:
- Monitor oxygenation, ventilation, and circulation continuously
- Caution in patients with hypotension, hypovolemia, or lipid metabolism disorders
- Risk of Propofol Infusion Syndrome (PRIS) with prolonged use in ICU
Adverse Reactions:
- Common: Hypotension, bradycardia, apnea, injection site pain
- Serious: Respiratory depression, metabolic acidosis, PRIS (in rare cases)
Drug Interactions:
- CNS depressants (opioids, benzodiazepines): Enhanced sedative effects
- Muscle relaxants: May increase neuromuscular blockade
- Alcohol and anesthetics: Additive CNS depression
Overdose & Management:
- Symptoms: Severe hypotension, respiratory depression, cardiovascular collapse
- Treatment: Discontinue propofol, provide airway support, oxygen, fluids, vasopressors if needed
Storage Conditions:
- Store between 2°C-25°C
- Do not freeze
- Use within 6 hours of opening
