This product is a colorless clear liquid.
This product is a local anesthetic and antiarrhythmic drug. It is mainly used in infiltration anesthesia, epidural anesthesia, surface anesthesia (including mucosal anesthesia during thoracoscopy or abdominal surgery) and nerve block. It can be used for ventricular premature beat and ventricular tachycardia after acute myocardial infarction, as well as for digitalis poisoning, cardiac surgery and ventricular arrhythmia caused by cardiac catheters. This product is usually ineffective for supraventricular arrhythmias.
Usage and dosage:
1. Anesthesia
(1) Adult dosage:
Surface anesthesia: 2% ~ 4% solution does not exceed 100mg at a time. The dose should not exceed 4.5mg/kg once (without epinephrine) or every 7mg/kg (with 1:200,000 concentration of epinephrine).
Sacral canal block for labor analgesia: 1.0% solution, limited to 200mg.
epidural block: thoracolumbar segment with 1.5% ~ 2.0% solution, 250 ~ 300mg.
Infiltration anesthesia or intravenous regional block: 0.25% ~ 0.5% solution, 50 ~ 300mg.
Peripheral nerve block: brachial plexus (unilateral) with 1.5% solution, 250-300mg; Dental 2% solution, 20 ~ 100mg; The intercostal nerve (each branch) was treated with 1% solution, 30mg, 300mg limit; Paracervical infiltration with 0.5% ~ 1.0% solution, left and right side of 100mg; Paravertebral spinal nerve block (each branch) with 1.0% solution, 30-50mg, 300mg limit; Pudendal nerve with 0.5% ~ 1.0% solution, left and right side of 100mg.
Sympathetic ganglion block: cervical stellate nerve with 1.0% solution, 50mg; Lumbar anesthesia with 1.0% solution, 50 ~ 100mg.
The one-time limit is 200mg (4mg/kg) without adrenalin and 300-350mg (6mg/ kg) with adrenalin; Intravenous regional block, maximum dose 4mg/kg; For treatment, the initial dose is 1 ~ 2mg/kg, the maximum dose is 4mg/kg, and the limit of intravenous drip for adults is 1mg per minute. Repeated administration, the interval should not be less than 45 to 60 minutes.
(2) Common dosage for children
Depending on the individual, the total amount of one administration should not exceed 4.0 ~ 4.5mg/kg, 0.25 ~ 0.5% solution is commonly used, and 1.0% solution is used in special cases.
2. Anti-arrhythmia
(1) Common dosage: Intravenous injection 1 ~ 1.5mg/kg body weight (generally 50 ~ 100mg) for the first load of 2 ~ 3 minutes, if necessary, repeat intravenous injection 1 ~ 2 times after 5 minutes, but the total amount within 1 hour shall not exceed 300mg. Generally, 5% glucose injection is mixed into 1 to 4mg/ml drug drops or administered by infusion pump. After using the load, it can be continuously maintained by intravenous drip at a rate of 1 ~ 4mg per minute, or by intravenous drip at a rate of 0·015 ~ 0.03mg/kg body weight per minute. The elderly, heart failure, cardiogenic shock, reduced liver blood flow, liver or kidney dysfunction should reduce the dosage, at 0.5 to 1 mg per minute. It can be administered intravenously with 0.1% solution, not exceeding 100mg per hour.
(2) Maximum dose: The maximum load within 1 hour of intravenous injection is 4.5mg/kg body weight (or 300mg). Maximum maintenance is 4mg per minute.
Matters needing attention:
(1) When non-intravenous administration, it should be prevented from straying into blood vessels, and pay attention to the diagnosis and treatment of local anesthetic poisoning symptoms.
(2) During medication, pay attention to check blood pressure, monitor electrocardiogram, and have rescue equipment; Ecg P-R interval prolongation or QRS widening, other arrhythmias or aggravation of the original arrhythmia should be stopped immediately.
