Description:
Spironolactone is a potassium-sparing diuretic that works by blocking aldosterone, a hormone that regulates salt and water balance. It is used to treat high blood pressure, heart failure, and conditions associated with excessive aldosterone production.
Indications:
Spironolactone Tablets are indicated for:
- Edema due to congestive heart failure, liver cirrhosis, or nephrotic syndrome
- Hypertension (high blood pressure)
- Primary hyperaldosteronism (Conn's syndrome)
- Hypokalemia (low potassium levels)
- Heart failure (NYHA Class III-IV) to reduce morbidity and mortality
Dosage & Administration:
Adults:
- Edema & Hypertension: 25-100mg once daily, adjusted as needed
- Severe Heart Failure: 25mg once daily, up to 50mg/day if tolerated
- Primary Hyperaldosteronism: 100-400mg daily (before surgery or long-term treatment)
- Hypokalemia: 25-100mg daily
Pediatric Use:
- Edema & Hypertension: 1-3mg/kg/day in divided doses
Renal Impairment:
- Use with caution in mild to moderate kidney disease
- Contraindicated in severe renal impairment (eGFR <30 mL/min/1.73m²)
Hepatic Impairment:
- Use with caution in liver disease, as fluid-electrolyte balance is critical
Administration:
- Take once daily, preferably in the morning
- Can be taken with or without food
- Do not crush or chew
Contraindications:
- Hypersensitivity to Spironolactone or excipients
- Severe kidney disease or anuria
- Hyperkalemia (high potassium levels)
- Addison's disease (adrenal insufficiency)
- Concomitant use with potassium supplements or potassium-sparing diuretics
Precautions:
- Monitor potassium levels regularly (risk of hyperkalemia)
- Caution in diabetic patients (higher risk of hyperkalemia)
- Risk of gynecomastia (breast enlargement in men) with long-term use
- Use with caution in elderly patients due to increased risk of electrolyte imbalance
- May cause dizziness-avoid driving or operating machinery
Adverse Reactions:
- Common: Dizziness, headache, nausea, menstrual irregularities
- Electrolyte Imbalance: Hyperkalemia, hyponatremia
- Hormonal Effects: Gynecomastia, breast tenderness, irregular menstruation
- Serious (rare): Severe hyperkalemia, kidney dysfunction, metabolic acidosis
Drug Interactions:
- ACE inhibitors & ARBs: Increased risk of hyperkalemia
- NSAIDs: May reduce diuretic effect and worsen kidney function
- Lithium: Increased risk of lithium toxicity
- Digoxin: May increase digoxin toxicity risk
- Other diuretics: Increased dehydration and electrolyte disturbances
Storage Conditions:
